Fat grafting is a procedure that is frequently performed in combination with other cosmetic surgeries/procedures, such as upper eyelid surgery, lower blepharoplasty, brow lift, mid-face lift, face/neck lift, and CO2 laser resurfacing.

In particular, Dr. Lewen regularly uses facial fat transfer to achieve a more balanced and complete rejuvenation of the lower eyelids for certain patients in combination with blepharoplasty surgery. For these patients, an optimal rejuvenation of the lower eyelids cannot be accomplished by just simply removing the bags of fat or pulling the lower eyelid skin tighter. It is important to understand the significance of volume, shape, and positioning of the midface when considering the best rejuvenating options for the lower eyelids. It is this relationship between the lower eyelids and the midface, along with the smoothness of transition between these two areas of the face, that is integral for the best possible cosmetic rejuvenation with natural-appearing results. For this reason, the ideal rejuvenation of the lower eyelids for many patients may necessitate some combination of the following procedures:

• Lower blepharoplasty to sculpt and reposition the fat pockets that may be causing the appearance of “bags” under the eyes
• A re-suspension of the muscle tissue in the lower eyelid, lifting the upper portion of the midface and the transition between the lower eyelid and the midface into a more youthful position
• Tightening the lower eyelids to secure and maintain a youthful and rejuvenated shape to the eyes
• Tightening the skin underneath the eyes to minimize the appearance of wrinkles or folds under the eyes
• Volume replacement in the lower eyelid or midface with facial fat grafting or facial filler injections to create a smoother transition between the lower eyelid and the midface tissues

Fat transfer and injections require some downtime, depending on the area treated and the extent of the treatment performed. The area where the fat was harvested and the treatment area may feel tender and you may experience some minor swelling.
