Consultation Fee
The consultation fee is non-refundable and collected at the time of scheduling to reserve and protect your time on Dr. Lewen’s schedule and allows our office to devote the appropriate amount of time to addressing your concerns. Every consultation is scheduled for 1 hour to allow for significant dedicated time with Dr. Lewen.
Consultation pricing
Our Consultation Fee is $400.00 USD, which is applicable towards certain procedures discussed with Dr. Lewen.
Our Revision Surgery Consultation Fee is $650.00 USD, which is applicable towards certain procedures discussed with Dr. Lewen.
Our Consultation Fee with Dr. Bloom is $150.00 USD, which is applicable towards certain procedures discussed with Dr. Bloom.
Our Consultation Fee with Nicole, Nurse Practitioner is $150.00 USD, which is applicable towards certain procedures discussed with Nicole.
